This data set includes comparative cost and quality information for clinics with five or more physicians for medical claims in 2016. Only clinics with eligible Total Cost of Care indices and three quality measures, Breast Cancer Screening, A1c testing, and Medical Attention for Nephropathy are included.

Centers for Medicare Services began reducing Medicare payments for subsection (d) hospitals that rank in the worst performing quartile with respect to hospital-acquired conditions (HACs). Hospitals with a Total HAC Score above the 75th percentile of the Total HAC Score distribution may be subject to the payment reduction. This table contains a hospital’s measure, domain and Total HAC scores. The measure and domain scores comprise the Total HAC Score.

A provider level summary of Outpatient Prospective Payment System (OPPS) average estimated submitted charges and average Medicare Payments for 30 selected Ambulatory Payment Classification (APC) Groups. 9/04/2013 UPDATE: As a result of using the OPPS ratesetting process to develop the summary of outpatient charge and payment data, we inadvertently excluded claims data for visit APCs 0604 (“Level 1 Hospital Clinic Visits”) and 0607 (“Level 4 Hospital Clinic Visits”). The revised outpatient data reflects changes to correct this, with volume increases in the summarized data for APCs 0604 and 0607.

This data set contains nursing home deficiencies noted by Medicare for facilities in Utah. The data contains addresses, a list of the finding and a date of the correction.
