- Health Small Statistical Areas are the 63 small health statistical areas in Utah. These boundaries were developed by the Utah Department of Health by combining neighboring ZIP code areas within a county to create 63 small areas of approximately 33,500 persons.

- Time windows used as inclusion and exclusion criteria for quality measures may vary across measures. HEDIS measure definitions are subject to periodic review and change by NCQA. APCD is processed semiannually and data is subject to change as new data become available.
- Limitations
- APCD does not receive claims for Medicare Part A or Part B. Results may be skewed for measures with inclusion criteria for age ranges 65 and over.

- The quality measures are based on Healthcare Effectiveness Data and Information Set (HEDIS) specifications which are maintained by the National Committee for Quality Assurance (NCQA).
